The format used for date fields in the form will be based on your browser or device settings. To format the date in the response summary, email, or confirmation message, you can update your locale settings.

Login to Formfacade > click on the form to open it > Edit page will be displayed > click on the form title to select it > click on the gear icon next to the form title > Form settings page will be displayed > click Advanced > update the Locale details and click Save.

Note: English and English (US) refers to the US locale with mm/dd/yyyy date format. If you would like to use a different date format, you can choose the respective locale. I'd recommend you to use the Chrome browser to update the language settings so that all these locales are displayed in the Locale option in the Formfacade Form settings page.
