Hi Chad, If you just need to display the full name and date of birth in each page of your form, then you can click on the page title to select it and use @ option in the description editor (see screenshot below) to include the relevant details.

If you would like to record the answers in each of the pages, then you can use the Calculate option. In the Formesign Edit page, select the title for the second page > click on the + Add field icon > enter the title "Full name", select Short answer question > click on the settings gear icon > Question settings page will be displayed > click Answer > type @ in the Calculate option and select the client full name field from the list > click Save. Repeat the steps to autofill the date of birth.
