Hi Daryl, you have deleted the calculated score field in the form and selected the "Which of the following symptoms apply ..." grid question to set up the conditional message. That's why the form is not working now. Please follow the instructions to set up your form. Calculate score Login to Formfacade > click Forms > click on the form to open it > In the Formfacade Edit page, click on the last question in the form > click on the + Add field icon > enter the question title "Total Score", select Short answer question type > click on the settings gear icon > Question settings page will be displayed > click Answer > enter POINTS() formula in the Calculate option > select Hidden for the Appearance option and click Save. Configure conditional message Login to Formfacade > click on the form to open it > Edit page will be displayed > In the Edit page, scroll to the submit section > click Configure > on Submit settings page will be displayed > "Show conditional message based on score" option will be selected > scroll down and click on the Edit button > select the "Total Score" field for the "Conditional message based on" option and click Save > click Save to complete the set up. You can then submit a response to test the form.
