Open forms.google.com > search and click on the form to open it (OR) login to Formfacade > click on the form to open it > Edit page will be displayed > click on the form title to select it > click on the gear icon next to the form title > Form settings page will be displayed > click Advanced > click Launch to open the form in Google Forms.
