Hi Helene, Login to Formesign. Click on the form to open it. Edit page will be displayed. In the Edit page, click on the profile name in the top right corner. Click Change theme. Theme settings page will be displayed in the Preview page. Click Page. Enter the required text in the Title (header), Footer option (replaces "This site belongs to...") and click Save. You can upload your logo (square image 1:1 aspect ratio) to replace the default profile picture.
