Hi Han, you must disable the shuffle question order option in Google Forms and sync the changes with Formfacade.
Open your form in Google Forms > click Settings > click Presentation > disable Shuffle question order option > click on the addon icon > click Formfacade > click More options > click Update or Customize to sync the changes with Formfacade and then test your form. Please try this and confirm.
