Login to Formfacade. Click on the form to open it. Edit page will be displayed. In the Formfacade Edit page, click on the profile name in the top right corner. Click Change theme. Theme settings page will be displayed in the Preview page. Click Page. Enter the required text in the Title (header), Footer option (replaces "This site belongs to...") and click Save. The theme settings are for your account. It will applicable for all the forms you create in your account. The title will set as your account name in Formfacade.
