7.1 Nature of Services

I am enrolling in a program that may include strenuous physical activity and dietary/lifestyle changes, including but not limited to:

Personal training
Semi-private training
Large group training
Boxing exercise classes
Athletic performance training
Online coaching and training
Nutrition coaching
Medical nutrition therapy

7.2 Medical Consultation

The SVNP Team recommends you consult with your physician before engaging in this program to ensure your safety and health.

7.3 Assumption of Risk

I assume all risk associated with the dietary, exercise, or lifestyle changes I will adopt. I understand that I may experience possible injury or death, including but not limited to:

Muscle strains, sprains, pulls, or tears
Bruises, cuts, or abrasions
Joint, ligament, or tendon injuries
Broken bones
Cardiovascular events such as heart attack or stroke
Loss of balance or falls
Accidental collisions with other participants or equipment
Other serious injuries, including potential disability or death

7.4 Health Certification

I hereby affirm that:

I do not suffer from any condition that would prevent my participation
I have not withheld any information regarding my current health condition
I certify that I am in good health
I acknowledge that I have consulted with a physician or chosen to participate at my own risk

7.5 Special Medical Conditions

If I am pregnant, or have high cholesterol, high blood pressure, diabetes, renal disease, or other medical conditions requiring special dietary restrictions, or special modifications to training, I must receive permission from my physician before participating.

7.6 Allergies and Medications

I have disclosed all food allergies/sensitivities during the initial consultation
I have specified all prescription and OTC medications and discussed side effects with my doctor or pharmacist

7.7 Physical Touching

I understand that certain physical touching may be necessary for demonstration and correction of exercise technique and alignment. If I do not want to be touched, I will make it known ahead of time.

7.8 Ongoing Responsibility

I acknowledge that if my health changes or if I do not feel comfortable at any time, it is my responsibility to inform my trainer, dietitian, and/or nutritionist and seek medical advice.

7.9 Release of Liability and Hold Harmless

I have volunteered to participate in SVNP programs. In consideration of their agreement to assist me, I do here and forever release and discharge and hereby hold harmless Silicon Valley Nutrition and Performance LLC, Evolution Sports Expo LLC, and their respective officers, employees, trainers, dietitians, nutritionists, instructors, agents, heirs, assigns, contractors, and affiliates from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with my participation in any SVNP Program, including any injuries resulting therefrom.

I further agree to indemnify and hold harmless Silicon Valley Nutrition and Performance LLC and Evolution Sports Expo LLC from any claims made by third parties arising from my participation.

7.10 Medical Consent

In the event of a medical emergency, I authorize SVNP and its staff to seek medical attention on my behalf if I am unable to do so. I understand that I am responsible for all medical expenses incurred.
