Please note: There are TWO parts to the Scrip process

Part 1: Google Form- Fill out the following Google Form to indicate WHO you want your donations to go to.

Part 2: PAY (via PayPal or check) for the donations you intend to make in this Google Form. After completing the Google Form, go to PayPal  and enter the total amount from this Google Form and PAY via PayPal, the link is also provided at the end of this form.  If paying by check, please turn a check made out to "SVE PTA" into the office by Thursday, November 21st.

**You should receive a confirmation email from PayPal once your donation is complete.**
