Note: To be accepted, photos must meet outlined requirements. If easier, Willow Glen Electric will be on-site in late April/early May to inspect and take photos of sub-panels (this service is provided at no cost to members). Members MUST verify their panel via this form or be available for inspection by Willow Glen. Thank you!

**Regardless of what type of panel you have (Zinsco or upgraded), upload a photo of your open electrical sub-panel here.** Photos must be clear, with the panel door open, the manufacturer label and your unit number (on a sticky note) visible. *Photos that do not meet these requirements, or which have been edited, will not be accepted by our insurer. In that case, Willow Glen Electric will need to inspect your panel.\*
