This process is designed to ensure you receive, complete, and submit your required task efficiently. Follow these steps precisely to ensure your documents are submitted correctly:

📝 Part 1: Download and Complete

When you click the links below, each file will automatically begin downloading to your device.

Click the Link: Click the link for each document below.
Confirm the Download: A prompt may appear asking you to confirm the download. Click "Save" or "OK." The file will now download.
- Download Location: By default, the file will save to the "Downloads" folder on your computer, tablet, or phone.
Locate the File:
- On Windows/Mac: Go to your "Downloads" folder (or the location you have set for downloads).
- On Mobile Devices: Check your device's file manager or the notification panel for the completed download.
- The files will all be DOC or DOCX that can be opened by most standard office software (e.g., Microsoft Word, Google Docs).
- Open and Complete: Double-click the file to open it. Read all instructions within the document carefully and complete all required tasks.
Save Your Completed Documents:
- Go to File > Save As (or File > Export).
- Crucially, rename the file to include your Surname, for example: SMITH_Secondary Health Declaration Form.docx.
- Save it in an easily accessible location on your device (e.g., your Desktop or a dedicated folder).

⚙️ Part 2: Upload and Submit

Locate the Submission Area: Scroll down this page to find the add file button.
Click to Upload: Click the "Browse Files" button.
Select Your File: Navigate to the location where you saved your renamed and completed file (e.g., SMITH_Secondary Health Declaration Form.docx).
Confirm Upload: Select the file and click "Open." Wait for the file name to appear in the submission box.
- Click "Add More" if you are uploading several documents, or else click "Done".
Final Submission: Click the "Submit" or "Upload" button at the bottom of the page to finalise your submission. You should receive a confirmation message.

⚠️ Important Notes

Do not complete the file without saving it. Unsaved changes will be lost.
Do not upload the blank, original downloaded file. Ensure you upload the file with your completed details.
