>> STATE OF CALIFORNIA SELLERS PERMIT

If you DO NOT already hold valid annual permits for the following uses, temporary permits may be purchased along with your booth rental during checkout:

>> CITY OF LAKE ELSINORE BUSINESS LICENSE

>> COUNTY OF RIVERSIDE ENVIRONMENTAL HEALTH ANNUAL PERMIT - MFF STICKER (Food Vendors Only)

If you cannot produce any or all of these documents now, you may skip this section and email the documents to stormhalloweenfestival@gmail.com within 7 days of submitting this registration form. Failure to do so risks automatic termination of registration and loss of selected booth location.
