Complete this entry form and submit payment - Fees must be paid at the time of registration. Your registration is not complete until the entry fees are received by Becky. Fees are not refundable. Cancellation policy: NO REFUNDS after close without a doctor/vet note, in which case fees will be refunded minus $35 office fee and any shirts.

Fees can be paid via PayPal (additional fee), Zelle (no cost) or check - at the end of the

registration form, select your payment method:

Fortuna, CA 95540 (mail check after you complete this form; total will be available at

checkout)

2. PayPal - this form will accept PayPal payment (fee will be included at checkout);

3. Zelle - you will need to pay via Zelle using the QR code provided at check out or

email address sierrapacifictreasurer@gmail.com . The total will be available at

checkout after you complete the form. 

Meals will be available for purchase through April 1st. Extra meals will be available on site to purchase in limited quanitities.

While entries are completed through this form, we do ask that you speak with your DC and club about potential teams (members, captains, chaperones etc) before signing up. We will ask for requested team members, captains and chaperones, and we will do our best to honor the request. But they are just requests, no guarantees.

During the entry process we will ask that you fill out these forms and upload copies (pictures of the form works). If you are unable to upload them, email them to teketales@gmail.com or bring them to the rally:

-Camelot Equestrian Park Waiver

-Team Chaperone Form (if you are interested in being a Team Chaperone - details within the form)

Please note in addition to those forms:

- All coaches will be required to fill out coaches form and bring to the rally. The coaches form is required in order for coaches to recieve their coaches badge.

-If the entry is for a minor and with permission that participant ends up on a senior team (a team with a member 18+) - this Junior/Senior team form is required to be completed by the parent/guardian prior to packet pick up. *you will indicate the preference within this entry*
