Downtown Doggy Daycare: New Dog Enrollment

STEP 1. Have you checked if we have availability for your requested service and dates? If not, please go to https://www.dmdoggy.com/reservation and submit the form. We will contact you shortly regarding availability and how to proceed with enrollment.

STEP 2. Complete Enrollment and Policy Agreement Forms. Once availability is verified, we will request you submit this enrollment form and our policy agreement form. Please fill out the following form if you are wanting to enroll your dog(s) into daycare, boarding or both. You can submit enrollment for up to 3 dogs. If you have 4+ dogs, please submit an additional form.

STEP 3. Submit your dog(s) vaccinations/fecal and spay/neuter verification documents. All dogs must have current vaccinations, proof of a fecal test and be on monthly heartworm and flea preventative. Owners must submit written proof of the Rabies, Distemper booster and Bordetella (kennel cough) vaccine along with a negative fecal test within the last year.

All dogs over the age of 6 months must be spayed or neutered. We must receive written confirmation of the surgery from your vet once this is done at the appropriate age.

You or your veterinarian can email vaccinations and other documents to info@dmdoggy.com.

STEP 4. Schedule a Sniff-n-Wag Evaluation: All dogs must be interviewed prior to a visit to see if they are a proper fit for the daycare setting. We will evaluate your pet by gradually introducing him or her to other dogs in their playgroup. A Sniff-n-Wag lets us guarantee a safe environment for all of our dogs.

Once your enrollment paperwork and vet records are received, we will email you to setup this evaluation.
