I, the parent/authorized guardian of the above child(ren)/teen(s), agree (by checking this box, I recognize that this is equivalent to my legal signature) to give my permission for my above mentioned child(ren)/teen(s) to participate in all children's and teen's activities and programs conducted by Prosper Fellowship Church (PFC) or its representatives either in the retreat's building or outside. I UNDERSTAND AND DO HEREBY AGREE TO ASSUME ALL OF THE RISKS AND OTHER RELATED RISKS WHICH MAY BE ENCOUNTERED BY MY SON/DAUGHTER PARTICIPATING IN THE ABOVE ACTIVITY. I agree that I hereby hold harmless and waive any and all claims against Prosper Fellowship Church, its staff, and leaders for any accident, bodily or personal injury, damage to or loss or theft of any property, illness, or death of any person, including without limitation demands, liabilities, damages, judgments, losses, costs, expenses and/or penalties, including attorneys’ and consultants’ fee and disbursements, which arise out of joining the activities/programs sponsored by PFC in this retreat. I further state that I HAVE CAREFULLY READ THE FORGOING RELEASE AND KNOWN THE CONTENTS THEREOF AND IS SIGNING THIS RELEASE AS AN ACT OF MY OWN FREE WILL. This is a legally binding agreement, which I have read and understand. I also give my permission for my child(ren)/teen(s) to be photographed, videotaped, and/or interviewed by representatives from PFC for the purpose of publications, which include church bulletins, promotions, website, online media, social platforms and church presentations. I fully relinquish my right or interest in any film, tape, or photograph which may be used for any legitimate purpose.
