We are thrilled to have you participate in our youth tackle football program with the  Desert Devils football and Cheer Organization . Before you begin your involvement with our program, we require that you and your parent or guardian sign this waiver of liability. 

As the parent or legal guardian of the participant,I fully understand that participating in football can be a potentially hazardous activity. I recognize that there are risks involved, including, but not limited to, serious head and bodily injuries, paralysis, and even death. 

I, as the parent or legal guardian of the player, waive and release Desert Devils football and Cheer, its officers, coaches, and volunteers from any and all liability for any injury, loss, or damage sustained by the player during their participation in the program. I understand that this waiver and release extends to all claims, whether caused by the negligence of the organization, its officers, coaches, or volunteers, or otherwise.

I, the player, understand the risks involved in participating in Desert Devils youth tackle football program. I acknowledge that I am participating voluntarily and with knowledge of the dangers involved. I agree to assume any and all risks of injury or death and to release Desert Devils, its officers, coaches, and volunteers from any liability arising from my participation in the program.

By signing below, I certify that I have read and fully understand this waiver of liability and that I am signing it voluntarily. I also acknowledge that by signing, I am waiving certain legal rights that I or my child would otherwise have.
