I, hereby acknowledge that participation in soccer competitions and traveling abroad carries inherent risks and potential hazards. In consideration of being permitted to participate in the 2025 Soccer European Trip organized by the EuroSports Network Select Soccer Team Club ("EuroSports Network"), I voluntarily agree to release, waive, discharge, and hold harmless EuroSports Network, its officers, directors, employees, coaches, chaperones, agents, and any other persons involved with EuroSports Network from any and all claims, liabilities, damages, or demands arising from or related to any injury, including death, that may occur during the course of the trip.

I understand and acknowledge that EuroSports Network does not provide accidental insurance coverage for participants of the trip. I am solely responsible for obtaining adequate insurance coverage to protect against any injuries or losses that may occur.

I further understand that EuroSports Network coaches may act as chaperones for the group and may advise me of any breaches of local rules, regulations, and customs by my/our son/daughter. EuroSports Network reserves the right, in its sole discretion, to discontinue the participation of any player in the tour or any activity organized by EuroSports Network for any violation of EuroSports Network rules and regulations. In such cases, EuroSports Network may require the player to return home at their own expense, and no refund will be provided for any unused portion of the tour.

I acknowledge that I have read and understand the terms of this release form, and I voluntarily agree to its contents.
