I understand and agree to the following:

Volunteering & Opt-out Fee
As a parent/guardian, I will sign-up and volunteer in some way. Should I opt-out of volunteering, I agree to pay the $150 volunteer opt-out fee which is due at the time of the audition. I further understand that I will be charged $150 should I cancel my volunteer assignment for any reason.

Attendance/Tardiness I understand that prompt attendance to all scheduled rehearsals is mandatory. I will not involve my dancer(s) in any activity that conflicts with their responsibilities to the production of the Nutcracker. I understand a total of (1) absence for any reason will be accepted during studio rehearsals, provided I have notified 4SDA prior to the absence. I also understand my dancer(s) may have their part reduced or may be removed from the production if (2) or more absences and/or regular tardiness occurs. Multiple absences and regular tardiness seriously hinders the understanding, memorization, and timing of choreography thus effecting other dancers in the choreography, and the production as a whole. I understand that attendance during Performance Week is mandatory, without exception. Dancers absent during Performance Week will automatically have their part(s) in the production reduced or removed entirely. Refunds and credit are not given if a dancer's part is reduced or removed from the production.

Illness or Family Emergency In the event of illness or family emergency, I understand that it is my responsibility to contact 4SDA prior to the scheduled rehearsal. I further understand each absence is reviewed individually, and that more than one absence may result in a reduced role or removal from the production as laid out in the Attendance/Tardiness section of this contract form. I understand that all paid fees are non-refundable and that account credit may be considered, but is not guaranteed to be given for illness or family emercency.

Prompt Pick-up I will pick up my dancer(s) promptly after rehearsal. I understand that 4S Dance Academy and/or its agents will not be responsible for any dancer supervision other than during the rehearsal time. I further understand that I will be charged a late pick up fee, $1 for every minute, starting at (15) minutes after the end of my dancer's scheduled rehearsal (e.g. 15 minutes late = $15 fee, 20 minutes late = $20 fee).

Code of Conduct I agree to behave in a courteous manner toward teachers, staff, students and other parents/families. I understand that confronting, disparaging, and/or aggressive behavior toward any 4SDA faculty or staff, student, or family, will be grounds for my dancer(s) immediate dismissal from the production. I agree to review the code of conduct with my dancer(s) and understand that they must arrive on time, stay quiet and attentive during rehearsals, while also abiding by 4SDA's and the theater/rehearsal venue’s rules of conduct. I understand that I am responsible for my student's behavior and conduct, as well as any family, caregiver or guardian while at 4SDA and the theater/rehearsal venue. In addition I understand a single violation of the code of conduct will result in a review and possibly the removal of my dancer(s) from the production and that any paid fees will not be credited or refunded.

Image & Likeness Release I understand and agree that for promotional, publicity or commercial advertising purposes, 4S Dance Academy shall have the right to use a photograph, video, voice or other likeness of my student/dancer. I also understand that I can email jason@4sdanceacademy.com or call the studio at 858-385-1999 with any questions, concerns, or to opt-out.

RELEASE As additional consideration for the student's instruction and participation, the undersigned hereby releases and holds harmless 4S Dance Academy and/or any of its agents or representatives from any liability whatsoever arising out of injury to the person or property of the student which may occur while participating in the activities of 4S Dance Academy or its affiliated Company. The undersigned further agrees to indemnify 4S Dance Academy in the event any claims are asserted against it arising from the student's participation. I (we) understand and agree that in participating in any physical activity, there are risks for injury or bodily harm and agree to assume all such risks.

Participation in The Nutcracker is contingent upon adherence to the rules set forth in this contract and the Nutcracker Guidelines. The participation fee will not be refunded for any reason other than the dancer not being cast in the production.
