IEP/504 Meeting Support Agreement (PHP)

Agreement for Tier 2 and Tier 3 Families: Establishes expectations and defines the supportive role of PHP staff during the Special Education process meetings.

1.1. Non-Advocate/Non-Legal Status: I understand the PHP staff is not an attorney, cannot provide legal advice, or act as my legal representative.
1.2. Role of PHP Staff: I understand the PHP staff's role is to provide emotional support, help organize thoughts, take notes, and clarify terminology.
1.3. Parent-Led Communication: I understand I am the leader of the IEP team and the PHP staff member will support me, but will not speak for me or take over the meeting.

2.1. Mandatory Prep Meeting: I agree to participate in a scheduled Pre-Meeting with my assigned PHP staff member prior to the official school meeting.
2.2. Required Paperwork: I agree to provide PHP staff with all current and relevant paperwork (eligibility reports, evaluations, most recent IEP/504) prior to the Pre-Meeting.

3.1. Minimum Notice: I agree to provide PHP with a minimum of one week (7 days) notice prior to the scheduled school meeting.
3.2. Coordinated Scheduling: I understand PHP cannot guarantee attendance if the meeting was scheduled with the school without prior coordination with PHP staff.
3.3. Staff Substitution: I understand that if my assigned staff member is unavailable, PHP will attempt to send another qualified representative.
3.4. Notifying the School: I agree to inform the school in advance that a representative from PHP will be attending the meeting with me.

4.1. Professional Behavior: I agree to maintain a professional, respectful, and collaborative demeanor toward all school personnel and PHP support staff at all times.
4.2. Pausing to Regroup: I agree that I or my PHP staff member can request a pause/break if emotions escalate, and I agree to step out to regroup when prompted.
4.3. Termination of Future Support: I understand that if I exhibit unprofessional behavior AND deny a request to break, future PHP meeting support will be terminated.
4.4. Mandatory Advocacy Coaching: I understand that if my behavior is deemed unprofessional, I will be required to attend advocacy coaching before PHP considers future support.

5.1. Mandatory Debrief: I agree to participate in a Post-Meeting Debrief with my PHP staff member shortly after the meeting concludes.
5.2. Post-Meeting Documentation: I agree to provide PHP staff with copies of all paperwork from the school after the meeting (PWN, updated IEP/504, data).

6.1. Privacy: I understand and agree that PHP will keep all documentation and discussions strictly confidential, sharing information only within the PHP team for support purposes.
