1. I am of legal age (18 years or older), and I understand that the minimum age requirement for getting a tattoo is 18 years. Valid identification may be required.

2. I have carefully considered my decision to get a tattoo and fully understand the potential risks involved with the procedure.

3. I have disclosed any medical conditions or allergies to the tattoo artist before starting the procedure.

4. The design that has been agreed upon is exactly what I want, and I accept full responsibility for it.

5. Any changes made to the design during or after the stencil application will be subject to an additional fee.

6. The tattoo artist has explained all possible risks associated with getting a tattoo, including but not limited to infection, scarring, allergic reactions, dissatisfaction with results, and general discomfort during the process.

7. The area where my tattoo will be placed has been cleaned and disinfected according to standard hygiene practices by trained professionals.

8. All equipment used in this process will be single-use disposable needles that are properly sterilized before use in accordance with industry standards.

9. Proper aftercare instructions have been provided by my tattoo artist clearly outlining how to ensure proper healing of my new tattoo.

10. In case of any complications or issues arising from this procedure due to my negligence or failure in following proper aftercare instructions provided by my tattoo artist/studio/maintenance/cleaning procedures post-tattoo; Myk3 Tattoo / Mikaella Reyes shall not be held liable for any damages or additional expenses incurred as a result thereof

11.I consent voluntarily without coercion from Myk3 Tattoo/ Mikaella Reyes' staff members/tattoo artists/employees/representatives/agencies/suppliers etc., on whom no liability shall accrue
