I, the undersigned, authorize The Little Dog Grooming Company to groom my dog(s) listed above in my home environment or designated location. I understand that grooming services may include brushing, bathing, nail trimming, ear cleaning, and other standard grooming procedures.
Home Access & Utilities Consent
I acknowledge that grooming services will take place in my home. I give permission for The Little Dog Grooming Co. to enter my home, and use water and electrical outlets as necessary to safely and effectively groom my pet.
Health & Safety Acknowledgment
I confirm that my dog is in good heath and up-to-date on rabies vaccinations and agree to provide proof upon request. I confirm that my pet and home are flea free, and will disclose any known medical, behavioral, or physical conditions that may affect grooming services.
I understand that The Little Dog Grooming Co. reserves the right to refuse or stop a groom at any time if it is deemed unsafe for the pet or groomer.
I acknowledge that dogs may react unpredictably during grooming. While every effort will be made to ensure a safe and low-stress experience, behavior cannot be guaranteed.
I understand that grooming may occasionally result in cuts, scratch, or stress-related reaction (including but not limited to dryer sensitivity or panic.)
I understand that grooming may sometimes reveal or irritate an existing or previously undetected medical condition, and I agree that The Little Dog Grooming Co. is not liable for such occurrences.
I acknowledge that a matted coat increases the risk of injury, skin irritation and discomfort. The Little Dog Grooming Co. will take all precautions possible to complete the groom safely but cannot promise there will be no nicks or clipper irritation.
Special Services
I understand that ear plucking is available upon request; however, it may cause discomfort, irritation, redness, or minor bleeding.
I understand that coloring products do not have the same effect on all coat types and may vary on color shade or vibrancy. I acknowledge that depending on coat type dye may fade slowly but can be reset through coat removal.
I understand that hair coloring or creative grooming may stain fur, skin, or household surfaces. I release The Little Dog Grooming Co. from liability for any staining or variation in color results.
Behavior & Liability
I understand that additional fees may apply for dogs displaying difficult or unsafe behavior, including but not limited to biting, excessive moment, or equipment interference. Behavioral fees will range from $15-35 and are assessed at the discretion of The Little Dog Grooming Co. Fees may decrease as behaviors improve.
I understand that The Little Dog Grooming Co. specializes in working with hard-to-handle dogs and will use appropriate, humane handling techniques to ensure safety.
I release The Little Dog Grooming Co. from liability for injury or stress-related incidents that may occur during grooming, except in cases of gross negligence or willful misconduct.
I understand that for the safety and well-being of my dog and the groomer, The Little Dog Grooming Co. may stop the grooming session at any time if my dog becomes unsafe or overly stressed, even if the groom is incomplete.
Payment & Cancellation
Payment is due in full at the time services are rendered.
A minimum of 24 hours notice is required for cancellations; late cancellations may result in a fee of 50% of the cost of the initial scheduled services.
Late arrival or inability to access the home will result in the following fees:
First occurrence: $25
Second occurrence: $50
Third occurrence: Termination of future grooming services.
Emergency Procedures
In case of a medical emergency, I authorize The Little Dog Grooming Co. to seek veterinary care for my dog. I understand that I am fully responsible for any associated veterinary costs.

By signing below, I confirm that I have read, understand, and agree to the terms outlined in this agreement. I consent to the grooming services provided by The Little Dog Grooming Co.
