GUIDELINES AND QUALIFICATIONS FOR SCHOLARSHIP APPLICANTS:
*Applicant must be a graduating senior from a local high school in Clay, Duval, Putnam, or St. Johns County.
*Applicant must have a minimum cumulative grade point average of 2.5 or above.
*Submit documentation of church, community and/or civic involvement; UPLOAD within the application. Documentation must include volunteer hours within the last 24 months.
*Submit two letters of recommendation; UPLOAD within the application. Limit 2 pages for each, double-spaced, 12 pt. font; Must be from a school official, teacher, adviser, counselor, community leader, pastor, or other individual with knowledge of applicant's academic achievements and/or civic involvement...RECOMMENDATION CANNOT BE FROM A FRIEND OR RELATIVE.
*Submit official transcript; UPLOAD within the application
*Submit college/university acceptance letter; UPLOAD within the application
*Submit copy of official SAT and/or ACT scores (Super Scores accepted)-CLT; UPLOAD within the application
*Submit a current SENIOR YEARBOOK PHOTOGRAPH;
UPLOAD within the application
*Submit a 500-word essay based on one of the required topics;
UPLOAD within the application
Choose ONE of the three essay topics:
1. Describe the most significant challenge you have faced and the steps you took to overcome it. How has this challenge impacted your academic achievement?
2. How have you contributed to your community, and in what ways has this service shaped your personal growth and future goals?
3. Describe a situation in which you demonstrated leadership or initiative. What did you learn about yourself?
(Essay must be 500 words, typed, double spaced, and in 12 pt. font; Heading should include first and last name, essay topic and date)
