By signing this form, I consent to maintaining my EFT membership throughout Zenith’s temporary closure, currently estimated to last from December 2026 through mid-March 2027. I understand that this timeline is subject to change and that my membership will continue to be automatically billed on the 1st of each month during this period.

By continuing my membership during the closure, I will secure my current membership rate for 12 months following Zenith’s reopening. Membership rates will increase when the new facility opens; however, members who maintain their EFT memberships throughout the closure will retain their existing rates for one full year after reopening.

I understand that I may cancel my EFT membership at any time without penalty. However, canceling my membership will forfeit this rate-lock benefit, and any future membership will be subject to the rates in effect at that time.
