By completing this registration, I understand that I am voluntarily registering the above named child for full participation in Sideline Squad and its activities for the duration selected. Should my child require immediate or emergency medical care while engaged in an activity sponsored by Sideline Squad, in my absence, I hereby grant Sideline Squad authority to release my child for medical treatment to such medical personnel if I am unable to be reached or located at the tournament. In consideration for the privilege of allowing my child to participate in the club, I agree to release and hold harmless Sideline Squad, its members, officers, and staff, from any liability to or responsibility for bodily injury, damage, or illness to the above identified child while participating in any of the youth activities offered by Sideline Squad. Furthermore, I agree to indemnify and hold harmless Sideline Squad, its officers and agents with respect to any claim asserted by or on behalf of my child as a result of bodily injury, illness, or damage. In the event Sideline Squad does not feel like my child is ready to attend the camp and its activities due to maturity, behavior towards others, or safety, Sideline Squad will issue a full refund and have the child return to its guardian. I also hereby consent to any and all uses and displays by the releasees of the participant’s name, voice, likeness, image, appearance and biographical information in, on or in connection with any pictures, photographs, audio and video recordings, digital images that are shown on websites, in television programs and advertising, sales and marketing brochures, books, magazines, all other printed and electronic forms and media including without limitation for the purpose of promoting Sideline Squad and its mission.
