Each child will need to have two photos turned in. If you have more than one child, you will need to complete this form for each child. Thank you!
Photos Needed:
A baptismal photo of your child and
A current photo of your child.
You can upload your photos below. Photos should be .jpg or .png formats. Digital photos need to be crisp, clear and not too dark. After uploading your photos, please be sure to hit the submit button. You will receive a reply email confirming your submission. If you don't receive this email, check your spam/junk folder. If you do not see it in either of these folders, please contact Patty Lindner, Director of Communications, in the parish office at 513-489-8815 ext. 713 to be sure your photos were uploaded.
