Calculation of Attendance Infractions

- Absent, call-in within employees first 90 days : 1 Point

-Absent, call-in after PTO days are used, and no exception is met: 1 Point

-Absent three (3) consecutive days, Call-in after PTO/Sick Days are used, and no exception is met. 1 Point

-Tardy (less than 2 hours late): 1/4 Point

-Tardy (2 hours or more late): 1/2 Point

-Early Departure (Less than 2 hours): 1/4 Point

-Early Departure (2 or more hours): 1/2 Point


Disciplinary Attendance Infractions Yearly Point System

-3 Points: Verbal Warning

-4 Points: Written Warning, Possible Suspension
-6 Points: Employee Termination

*Employees must use all available PTO (Personal/Sick Hours) before taking unpaid time off or requesting vacation time for an absence.*

**Employees may be required to provide proper documentation stating the cause of an absence to the Personnel Department with-in 48 hours of an absence, unless physically and/or mentally impossible.**

*** Absences that arise unexpectedly must be received by 8:30 AM on the  day the employee is scheduled to work, if no absence form is is received within 30 minutes of the employee's scheduled shift start, that employee will be deemed a no-call no-show. *** 

**** Absence forms should be submitted when requesting time off that is within 2 weeks of the scheduled shift, along with a time off request. ****

**** Absence forms need to be accompanied by a request within NetSuite if you expect to use PTO/Vacation and should match the time off request put into NetSuite*****
