RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT ("AGREEMENT") In consideration of participating in any gymnastics or cheerleading activities, I represent that I understand the nature of this activity and that I am qualified, in good health, and in proper physical condition to participate in such activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that this activity involves risks of serious bodily injury, including permanent disability, paralysis, and death, which may be caused by my own actions or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the activity. I hereby release, discharge, and covenant not to sue Enterprise Gymnastics and Cheerleading, its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the activity takes place (each considered one of the “RELEASEES” herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the “releasees” or otherwise, including negligent rescue operations. I further agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost that they may incur as a result of such claim. I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it, and have signed it freely and without any inducement or assurance of any nature. I intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect. *

PARENTAL CONSENT: and I, the minor’s parent and/or legal guardian, understand the nature of the above-referenced activities and the minor’s experience and capabilities and believe the minor to be qualified to participate in such activity. In consideration for being allowed to enroll my child at Enterprise Gymnastics and Cheerleading I agree to comply with all policies and procedures as set forth below. I also understand that by signing below I a responsible for paying all tuition and fees charged to my account by EGC.

1. I understand that EGC is an ongoing program which has no scheduled end. *

2. I understand that I will be charged monthly tuition and fees until I officially withdraw my child by emailing (ENTERPRISEGYMNASTICSANDCHEER@GMAIL.COM) 15 days prior or I will be removed from class until I settle my account.

3. I understand that tuition and fees are due on the 1st of each month. I also understand that if payment is not received in our office by close of business on the 10th of the month a late fee will be charged to my account. In addition I understand that If I withdraw my child all tuition and fees are non refundable. *

4. I understand that I have a reserved class slot and therefore am responsible for paying all tuition and fees even if I do not attend all or some of the classes. *

5. I understand that if I miss a class a make up class is not guaranteed. A make up may be allowed if there is space and only if done during the month the original class was missed. Classes will not be over filled to accommodate make ups. *

6. I understand that in order to officially withdraw my child I must email (ENTERPRISEGYMNASTICSANDCHEER@GMAIL.COM) with my intent to withdraw 15 days prior to when my tuition is due. I also understand that I am still responsible for all tuition due prior to the date the withdrawal email was received.

7. I understand that if I do not pay my tuition and I do not show up for class for an entire month I will be administratively withdrawn from my class. I also understand that I will still be responsible for all tuition and fees due prior to the date I was administratively withdrawn. *

8. I understand that the registration fee is an annual fee and will be billed to my account each year on the month I enrolled. *

Enterprise Gymnastics & Cheerleading Parent Policies, Procedures & Acknowledgment This document outlines the policies and expectations for all athletes and families participating in Enterprise Gymnastics & Cheerleading. Code of Conduct Athletes: Show respect, follow safety rules, demonstrate good sportsmanship, and maintain appropriate behavior. Parents/Guardians: Support athletes positively, refrain from sideline coaching, and address concerns respectfully. Coaches/Staff: Maintain professionalism, prioritize safety, and provide constructive instruction. Sexual Abuse Prevention Policy Enterprise Gymnastics & Cheerleading is committed to providing a safe and abuse-free environment. We follow USAG SafeSport guidelines, prohibit private one-on-one interactions, require appropriate physical contact only for instruction and safety, and mandate reporting of any suspected misconduct. Anti-Bullying Policy Bullying of any kind is not tolerated. This includes verbal, physical, emotional, social, or cyberbullying. All reports will be investigated, and disciplinary action may be taken. Concussion Policy Any athlete suspected of a concussion will be removed from activity immediately and may not return without written medical clearance. Parents are responsible for monitoring symptoms at home. Gym Regulations Proper attire is required. Hair must be secured and jewelry removed. Athletes must arrive on time, respect equipment, and follow all safety instructions. Parents must remain in designated viewing areas. Program Policies / By-Laws Enterprise Gymnastics & Cheerleading is privately owned. Tuition is due regardless of attendance. Athletes are only supervised during their scheduled class time. Enterprise Gymnastics & Cheerleading is not responsible for athletes dropped off early or remaining after class unless enrolled in a supervised program. Management reserves the right to dismiss athletes or families for disruptive behavior. Policies may be updated at any time. Parent Acknowledgment & Signature I acknowledge that I have read, understand, and agree to comply with all policies, procedures, and regulations of Enterprise Gymnastics & Cheerleading. I understand that failure to comply may result in disciplinary action, including dismissal from the program.
