I understand and acknowledge the following:

 

1.       Supervision: I am aware that qualified staff from The Cottage OSHC will be accompanying the children while at The Cottage OSHC to ensure their safety and well-being. I understand The Cottage OSHC adheres to legal child staff ratios which are 15:1.

2.       Emergency Contact Information: I have provided accurate and up-to-date emergency contact information for the day of the excursion. I understand to keep my phone, to which the number I’ve provided in XAP, switched on in any case of required contact.

3.       Medical Information: I have disclosed any relevant medical information, allergies, or special needs that The Cottage OSHC staff should be aware of, and I have provided any necessary medications with clear instructions, including motion sickness.

4.       Activities: I acknowledge and grant permission for my child to participate in all activities that are included in the Vacation Care Day I have booked them in for, including water activities, the use of inflatables, public areas and various transport.

5.       Fit for Purpose: I can confirm my child is fit to attend every day I have booked them in for during Vacation Care. If my child is unwell or unfit in any way, I will not bring them to The Cottage OSHC.

6.       Behaviour: I understand if my child is not complying with Cottage staff, the child will be removed from the activity (including excursions) by means decided by The Cottage OSHC Responsible Person. I understand that I am responsible any additional costs due to this incident.

7.       Necessary Belongings: I understand the essentials I must provide my child with on each day they are attending The Cottage OSHC Vacation Care, including a hat, a drink bottle, enclosed shoes, a bag, a change of clothes, morning tea food, lunch food and specific essentials for Vacation Care days. I understand Cottage hats and/or wristbands will be supplied to all children throughout excursions. I understand that I will not provide my child with money to make purchases while under The Cottage OSHC care.

8.   Supervision: I understand The Cottage OSHC uses all necessary precautions to ensure the safety of the children and the staff will supervise the children closely and implement appropriate safety measures. I acknowledge that, despite the precautions taken, there may be inherent risks associated with activities.

I release and discharge The Cottage OSHC, its staff, and volunteers from any liability for any injury, loss, or damage arising out of my child's participation in the excursion, except to the extent caused by the negligence or wilful misconduct of The Cottage OSHC.
