2024 Ohio START Summit Awards Nomination

About the Awards
This year, Ohio START will be recognizing agency staff and partners for their outstanding service to Ohio families. The 2024 Ohio START Award winners will be announced during the 2024 Ohio START Annual Summit to be held May 2 at the Nationwide Hotel and Conference Center, 100 Green Meadows Dr. S, Lewis Center, OH 43035Below is information about who is eligible to receive each award.

Ohio START Caseworker of the Year: Recipient must be a current Ohio START caseworker serving families.

Ohio START Family Peer Mentor of the Year: Recipient must be a current Ohio START family peer mentor serving families.

Ohio START Supervisor of the Year: Recipient must be a current Ohio START supervisor with a dyad actively serving families.

Ohio START Community Partner of the Year: This person is an integral part of the local Ohio START team and collaborates with the team to meet the needs of families. This could include a therapist, case manager, treatment supervisor/administrator, court personnel, direct line or steering committee member.

About the Nomination Form
To complete the nomination, please include the following information:
  • Nominator, nominee, and supervisor contact info
  • Nominating letter
  • Recommendation letter from the county director (not required for Ohio START Community Partner of the Year)
  • Additional supporting materials (optional)

Nomination Deadline: March 4, 2024. All nominations must be submitted via this form. You will be notified of the outcome by the end of March.

Nominating and recommendation letters

In the nominating letter, explain why you believe the nominee deserves the award. Please keep in mind that the award reviewers are relying on the information you provide, as well as any supporting information, to choose the most deserving recipient. Clarity and specificity are most important. Identify examples of nominee’s activities that demonstrate the following characteristics:


  • Ability to develop positive relationships with the Ohio START team and families served.
  • Integrates the use of evidence-based practices when serving families affected by SUD and child maltreatment.
  • Commitment, leadership and initiative in breaking down barriers that impede effective service delivery, training and development of staff for excellence in service, or mending and building relationships with community partners on behalf of children, families and the agency.
  • Effective advocacy on behalf of staff, and the families and children.
  • Contributions reducing stigma related to families affected by SUD.
  • How did this person go above and beyond?

If you are nominating someone for the Ohio START Caseworker,  Supervisor, or Family Peer Mentor of the Year Award, at least one letter is required from the agency director.
