2025 Scholarship Application

Thank you for taking time to apply for an Alpha Clay Pearls, Inc. Foundation Scholarship. To ensure we have all the required information in the order needed for evaluation, please complete this application in full and follow the instructions outlined.

GUIDELINES AND QUALIFICATIONS FOR SCHOLARSHIP APPLICANTS:

● Applicant must be a graduating senior from a local high school in Clay, Duval, Putnam, or St. Johns County.

● Applicant must have a cumulative grade point average of 2.5 or above.

● Submit documentation of church, community and/or civic involvement.

Documentation must include some volunteer hours within the last twelve months.

● Submit two letters of recommendation.

Limit 2 pages for each, double spaced, 12 pt. font; Must be from a school official, teacher, advisor, counselor, community leader, pastor, or other individual with knowledge of applicant's academic achievements and/or civic involvement ... CANNOT BE A FRIEND, RELATIVE or EMPLOYER.

● Submit official transcript.

● Submit college/university acceptance letter.

● Submit copy of official SAT, ACT and /or CLT scores (Super Scores accepted).

● Submit official notification of the tuition and fees due date.

● Submit a current photograph.

● Submit a 500-word essay based on one of the required topics.

Choose ONE of the three essay topics:

• Describe a significant challenge you've overcome and how it has shaped your perspective on life and your future goals.

• Describe a person who has significantly influenced your life. How have they impacted your personal growth and future aspirations?

• Discuss a book, film, or piece of art that has profoundly impacted your worldview. How has it changed your perspective on life or society?

(Essay must be 500 words, typed, double spaced, and in 12 pt. font; Heading should include first and last name, essay topic and date)

Please complete the application and submit with all supporting documentation by the April 15, 2025 deadline. Incomplete applications will not be considered, and no applications will be received after April 15, 2025 11:59 p.m. An application is complete once the application and all supporting documents have been received. Scan and email the application and upload supporting documents with the submission of this form. No hand deliveries will be accepted.
