This Returning Independent Contractor Onboarding form will require you to acknowledge our agency policies and renew our Independent Contractor Agreement with you. Also, it will give you an opportunity to upload a new W9 form (if your tax info has changed and/or if a new form is requested by Bolt management) and/or enter new bank account & routing numbers for electronic payment processing (if your banking info has changed).
