I, the undersigned, am the parent or legal guardian of the minor under the age of 18 named below ("My Child"). I hereby irrevocably grant to New Deal Spotlights, its successors and assignees the right to record the likeness, performance, and/or voice of My Child on film, videotape, or digital media, to edit such film, videotape, or digital media at their discretion, to incorporate the same into a motion picture with the working title “New Deal Spotlights Promo” and to use or authorize the use of such film, videotape, and digital media or any portion thereof in any manner or media at any time or times throughout the world in perpetuity and to use My Child's likeness and voice in connection therewith, including promotion in all media.

I hereby release New Deal Spotlights and anyone using said film, videotape, digital media, or other material from any and all claims, damages, liabilities, costs and expenses which I or My Child now have or may hereafter have by reason of any use thereof.

I understand that the provisions of this release are legally binding.
