INSTRUCTIONS
Clare’s House Golden Leash assistance is intended for people who are recovering from a medical issue and due to injury or mobility challenge, need a little extra help. Clare's House volunteers provide dog walking to keep your furry friends happy, healthy, and at home where they belong. Maximum time for Golden Leash dog walking is three months. If the owner cannot resume walking their dog at that time, Clare's House will re-evaluate this case. Please allow up to two weeks for Clare's House to fully process and approve your application.
Before completing this application, please make sure you meet the following requirements for our Golden Leash program:
Owner must have a permanent residence, and be able to allow Golden Leash volunteers access to their home to take their dog for walks.
Veterinarian records: All vaccinations (rabies, distemper) must be current in order for us to offer dog walking for your pet. Dogs must be spayed/neutered and on preventative treatment for fleas/ticks. A copy of the Veterinarian record and a photo of your dog must be provided via email to: info@clareshousestl.org prior to Clare's House extending Golden Leash services.
Heartworm preventative is recommended for your dog's protection from exposure.
Dogs that are determined to be reactive/not dog friendly cannot be walked by Clare’s House volunteers.
