Welcome to HHCSS! We’re so glad to offer services to you (and your family if needed). Before we can start, we just need you to sign the Client Consent Form. This form gives us permission to provide the services you need.

What is the Client Consent Form?

It’s a simple document that explains a few important things:

• Understanding the Services: The form talks about what services we provide and why, including any risks and benefits. Signing it means you agree to the services after everything has been explained.

• Privacy and Confidentiality: Your personal information is safe with us. The form tells you how we protect your privacy and when we might need to share your info, like with your doctor or insurance.

• Permission to Provide Services: By signing, you’re giving HHCSS permission to provide you or your child with services.

• Sharing Your Information: Sometimes, we need to share your information with other professionals to make sure you get the best care. This form lets you decide if that’s okay.

• Your Rights: You have rights throughout your time with us, like the right to say no to services, the right to privacy, and the right to know about any changes to your care.
